Expiration of SEVIS DS-2019 (within 60 Days)

Our records indicate that your current J-1 DS-2019 is going to expire with the next 60 days. Your current program end is: 9 MAY 2009. In order to comply with J-1 regulations, you must notify our office of your future intentions.

A: If you plan to extend your period of study/research in the U.S.,

1. You must complete section A of the DISP International Student/Scholar Extension Request Form by going to www.disp.uconn.edu click ‘Forms’, click ‘Extension Forms’ and select the appropriate form for either students or scholars. During this time, you must continue to pursue completion of your studies on a FULL-TIME basis, if you are a J-1 student.

1. You must request your extension with required documents to our office BEFORE the above mentioned DS-2019 end date. Our processing time is generally two weeks; therefore please make the request in a timely fashion. A new SEVIS DS-2019 will be issued to you with the requested extension date indicated on the Extension/Departure Notice.

B: If you are leaving the U.S.,

1. Please complete the DISP International Student/Scholar Departure Notice by going to www.disp.uconn.edu click ‘Forms’, click ‘Departure Forms’and return it to our office AS SOON AS POSSIBLE.
